Study to Determine the Aetiology of Chlormethine Gel Induced-skin Drug Reaction in Early Stage Mycosis Fungoides Cutaneous T Cell Lymphoma (MF-CTCL)

Adult patients with early stage MF-CTCL (stage IA-IB) will be eligible for this study. A total of 100 early stage MF-CTCL patients diagnosed in the past year will be enrolled.
Treatment with CL gel will be applied once daily to all skin areas affected by MF-CTCL and, for 8 weeks, one selected skin area unaffected by MF-CTCL (0.5% body surface area) until treatment response (complete response), study treatment duration completed (56 weeks), progression, or another withdrawal criterion is met.
Depending on the type of skin drug-related reaction (if any) occurring after application of CL gel, this study will categorize patients into three different groups corresponding to three different treatment patterns:
* Group A: Patients with no skin drug reaction with CL gel application
* Group B: Patients developing a skin drug reaction of any grade with CL gel application, not due to allergic reaction to CL gel, will continue treatment at reduced application frequency
* Group C: Patients from Group B unable to tolerate reduced CL gel application frequency will apply a potent topical steroid twice daily in addition to CL gel applied every other day

An Exploratory, Single-centre, Two-part Study to Describe Mycosis Fungoides Characteristics and Explore Novel Biomarkers With a Multi- Modal Patient Profiling Approach by Comparing MF Patients to Healthy Volunteers

Mycosis fungoides (MF) is an ultra-orphan disease of which the etiology remains unknown. MF is diagnosed by correlating clinical appearance with histopathological analysis of often multiple invasive skin punch biopsies. To move patient care and the development of novel treatments for MF forward, objective, sensitive and reliable tools that are preferably non-invasive are desired. Therefore, the objective of the current study is to phenotype the early stages of mycosis fungoides in detail and to assess the response of chlormethine (CL) gel monotherapy. With this approach the investigators aim to detect novel biomarkers and to establish methodologies for the (non-)invasive monitoring of MF.

Salt form and phosphate modification of an SN-38-nitrogen mustard conjugate: Overcoming water solubility limitations, combined efficacy, and broadened antitumor Spectrum

Cancer therapy has long faced a critical bottleneck wherein monotherapy often proves insufficient due to tumor heterogeneity and compensatory mechanisms, while combination chemotherapy frequently leads to localized drug ratio mismatch and exacerbated toxicity owing to differential in vivo drug distribution. To fundamentally address this challenge, we focused on achieving precise combined therapy at the single-molecule level. The core of this strategy involves designing novel bifunctional molecules that integrate the Topo I inhibitor SN-38 and the alkylating agent nitrogen mustard via a covalent linkage, with further prodrug modification using salt forms and phosphate ester groups to optimize their physicochemical properties. Among the candidate molecules, MN33-47 and MN33-63 demonstrated outstanding performance, not only significantly improving the solubility of SN-38 but also exhibiting broad-spectrum antiproliferative activity superior to individual drugs or their physical mixtures across various cancer cell models. Mechanistic studies further revealed that these molecules inhibit Topo I and promote its degradation, while simultaneously inducing DNA crosslinking, thereby combinedly enhancing DNA damage. This cascade of events subsequently leads to G₂/M phase cell cycle arrest and inhibition of cell migration. Subsequent downregulation of Bcl-2 protein relieves its suppression of the mitochondrial apoptosis pathway, ultimately activating caspase-3 and effectively initiating the apoptosis program. In CT26 tumor-bearing mouse models, MN33-47 and MN33-63 exhibited significant dose-dependent tumor suppression without causing apparent toxic side effects, demonstrating superior safety compared to combination chemotherapy. In summary, this work provides a practical solution to overcome the clinical limitations of SN-38 and nitrogen mustard, primarily through molecular innovation that "solidifies" combination therapy into a single entity, thereby directly resolving the inherent conflict between enhancing efficacy and exacerbating toxicity.

Nitrogen Mustard Disrupts Bioenergetics and Activates Oxidative Stress-Induced Cell Death Pathways in Human Keratinocytes

Nitrogen mustard (HN2) is a highly reactive bifunctional alkylating agent that causes severe skin injury. To define its impact on keratinocyte bioenergetics and stress responses, we examined mitochondrial function, metabolism, and cell death signaling in human HaCaT cells. Seahorse analysis showed that HN2 caused time- and concentration-dependent suppression of oxidative phosphorylation, including reductions in basal, adenosine 5'-triphosphate-linked, and maximal oxygen consumption. Glycolytic activity was similarly impaired, with decreased extracellular acidification, reduced glucose-stimulated glycolysis, and loss of glycolytic capacity and reserve, indicating broad metabolic dysfunction. HN2-induced bioenergetic impairment triggered rapid nuclear accumulation of nuclear factor erythroid 2-related factor 2 (Nrf2) and upregulation of antioxidant and mitochondrial regulatory genes (HO-1, NQO1, GSTA4, and PGC1α). In parallel, HN2 activated multiple programmed cell death pathways, including apoptosis, autophagy, and ferroptosis, as evidenced by corresponding alterations in Bax, Bcl-xL, LC3-II, SQSTM1/p62, caspase-2, caspase-9, GPx4, TFRC, and ACSL4. Cell cycle analysis identified cells in G2/M as particularly susceptible to HN2, which exhibited enhanced apoptotic signaling. N-acetylcysteine attenuated Nrf2 activation, preserved mitochondrial and glycolytic function, and reduced activation of cell death pathways, demonstrating a central role for oxidative and electrophilic stress in HN2 toxicity. These findings reveal a novel mechanism by which HN2 disrupts keratinocyte bioenergetics to drive stress-dependent cell death and highlight antioxidant intervention as a potential strategy to mitigate HN2-induced skin injury.

Real-World Determinants of Olanzapine Prescribing for Antiemetic Prophylaxis in Adults Receiving Highly-Emetogenic Chemotherapy

Background::

Since 2017, guidelines recommend a four-drug prophylactic antiemetic regimen—5HT3 receptor antagonist (5HT3RA), NK1 receptor antagonist (NK1RA), dexamethasone, and olanzapine—for patients receiving highly emetogenic chemotherapy (HEC). This study examines prophylactic olanzapine prescribing in patients initiating HEC and associated sociodemographic, clinical, and contextual factors.

Methods::

A retrospective cohort study was conducted using electronic health record data from an academic cancer center and affiliated community practices across the state of North Carolina, joined with geocoded community factors. Adults ≥21 years initiating cisplatin, carmustine, dacarbazine, mechlorethamine, streptozocin, or cyclophosphamide plus anthracycline (AC) in 2022 were included. Antiemetic orders placed before HEC administration were classified as three-drug (5HT3RA, NK1RA, dexamethasone) or four-drug (plus olanzapine). Late olanzapine use (within 30 days post-HEC) was also assessed. Multivariable logistic regression evaluated factors associated with regimen type.

Results::

Among 878 patients, all received orders for the three-drug regimen, but only 300 (34.2%) had olanzapine ordered within 30 days before first HEC. After adjusting for covariates, olanzapine prescribing was less likely in patients with breast (OR = 0.13, 95%CI = 0.07-0.23), gastrointestinal (OR = 0.20, 95%CI = 0.04-0.94), gynecological (OR = 0.14, 95%CI = 0.03-0.69), and head and neck (OR = 0.17, 95%CI = 0.05-0.66) compared to hematological cancers, in those receiving cisplatin (OR = 0.20, 95%CI = 0.06-0.70) and carmustine (OR = 0.02, 95%CI = 0.00-0.48) compared to AC, and in community (OR = 0.40, 95%CI = 0.27-0.61) versus academic settings. Prescribing was also lower among patients residing in low- (OR = 0.49, 95%CI = 0.25-0.93) or lower-middle income (OR = 0.50, 95%CI = 0.32-0.78) areas, but higher among those living >6 miles from treatment facilities (OR = 1.95, 95%CI = 1.08-3.58). Among patients without prophylactic olanzapine, 12.5% were prescribed it within 1 to 30 days post-HEC.

Conclusions::

Despite guideline recommendations, prophylactic olanzapine use in HEC remains suboptimal, influenced by clinical and non-clinical contextual factors. Targeted efforts at information dissemination and updated prescribing systems are needed to promote equitable, evidence-based supportive care.

EULAR 2026大会于6月3日-6日在伦敦召开。作为 EULAR 的特色项目，WIN（What Is New）和HOT（How To Treat）聚焦各临床领域的最新动态，由业内顶尖专家主讲。 2026年 EULAR 邀请英国利兹大学的 Dennis McGonagle 教授带来主题报告《What is New in Psoriatic Arthritis?》。 Dennis McGonagle 教授的内容系统梳理了银屑病关节炎（PsA）在病理生理学、临床管理和未来治疗方向上的最新突破。病理机制 McGonagle教授回顾了1956年这一”欧洲新纪元”的起点——Wright发表了首篇关于银屑病关节炎的经典文献。70年后的今天，单细胞测序技术正在重写我们对PsA的理解，依托脊柱附着点单细胞图谱等前沿技术，学界对局部免疫微环境、细胞互作机制的认知实现重大突破。附着点微环境：间质细胞主导局部免疫稳态 1.调节性 T 细胞（Treg）分布缺陷：对比肠道、骨骼等组织，人体附着点内 Treg 细胞数量显著匮乏，先天免疫抑制能力不足，是附着点易发生持续性炎症的重要先天因素。 2.附着点成纤维细胞的免疫调控功能：研究共鉴定出 7 类附着点间质细胞群，其中附着点成纤维细胞具备类似 Treg 的免疫抑制表型，可通过 CD39/CD73 腺苷通路、间充质干细胞通路发挥抗炎作用，打破 “仅免疫细胞驱动炎症” 的传统认知。 3.CD200/CD200R1 核心抗炎通路：成纤维细胞高表达 CD200分子，可结合髓系细胞表面 CD200R1 形成抑制信号。动物实验证实：阻断 CD200 通路会显著加重关节炎症，激动 CD200R1 则发挥关节保护效应，该通路成为 PsA 全新治疗靶点。皮肤 - 关节轴：免疫细胞迁移的双重证据 IL-23 是 PsA 核心促炎因子，不同实验动物品系对 IL-23 过表达的应答存在显著差异：BALB/c 小鼠易出现典型 PsA 病变，C57BL/6 小鼠仅以皮肤银屑病为主，提示遗传背景决定皮肤炎症是否向关节侵袭。进一步机制研究发现：CCR2⁺髓系前体细胞可从皮肤定向迁移至关节、附着点组织，直接诱发关节炎症。人体样本通过体细胞线粒体突变溯源技术证实，同一患者皮肤与关节滑膜细胞存在共享线粒体变异位点，直接证明人体内存在皮肤来源细胞向关节的定向迁移，为 “银屑病先发、关节炎后发” 的临床规律提供分子证据。滑膜成纤维细胞表型差异通过滑膜成纤维细胞单细胞测序对比发现，PsA 与 RA 的滑膜成纤维细胞在 THY1、FAPα、VCAM-1 等标志物表达，以及 mTOR、YAP 信号通路激活模式上存在明显差异，为两种疾病的鉴别诊断、靶向药物差异化选择提供细胞层面依据。 IL-17 通路与人源化动物模型 IL-17是维持附着点炎症的核心通路，研究证实，抑制IL-17可明显减少附着点内T细胞、粒细胞浸润，有效缓解局部炎症。新型人源化PsA小鼠模型实现技术升级，该模型依托人类记忆T细胞与血清因子构建，可完整复刻PsA皮肤炎症、滑膜炎、骨破坏等多种临床表型，同时证实CD8⁺T细胞、血清活性因子共同参与疾病发生发展，也印证了PsA多通路、多细胞协同致病的特点，为新药筛选与机制研究提供了理想载体。临床实践新认知 JAK/TYK抑制剂：疗效悖论与安全风险 JAK/TYK 抑制剂作为口服靶向药物，在 PsA 临床应用广泛，但其悖论性免疫激活与血栓、心血管风险一直是当前临床用药核心关注点。 01 附着点髓系细胞悖论性激活 JAK1、TYK2是IL-10抗炎信号的关键节点，药物可有效抑制适应性免疫，下调IL-17、TNF-α等促炎因子；但同时会阻断IL-10对CD14⁺单核/巨噬细胞的负向调控，在LPS、TLR4等刺激下，髓系细胞会异常活化，大量分泌IL-23、促炎因子，反而加剧局部炎症。 02 心血管与血栓安全风险基于22000例PsA患者的TriNetX数据库长期随访数据显示，随访2~5年期间，JAK抑制剂组静脉血栓、心脑血管不良事件发生率（8.5%）远高于多数生物制剂（4%）。体外实验进一步证实，JAK抑制剂联合LPS可加速纤维蛋白凝集，明确了血栓风险的分子机制，提示临床对高龄、肥胖、血栓高危人群需谨慎选用。 03 新型 TYK 抑制剂临床潜力选择性 TYK2 抑制剂 ZasocitinibⅡ 期临床数据：ACR20、ACR50、ACR7 应答率显著优于安慰剂，凭借口服便捷性，为无法接受注射制剂的 PsA 患者提供新选择。同时报告梳理全球口服靶向药物研发管线，涵盖 TYK2/JAK1 双抑制剂、PDE4 抑制剂等多款在研小分子药物。新兴治疗策略相较于传统单抗，纳米抗体、多肽分子体积更小，组织穿透性更强，可抵达血供匮乏的附着点深部组织。目前靶向IL-17A/F、IL-23的多肽药物、纳米抗体陆续进入临床试验，其中双靶点纳米抗体Sonelokimab（靶向IL-17A/F）、新型IL-17A抑制剂Izokibep均展现出良好的疗效与安全性，为难治性附着点炎提供新方案。肥胖、高BMI会增加关节机械负荷，加重全身炎症负担，是PsA病情活动的重要诱因。以GLP-1/GIP双受体激动剂（替尔泊肽）为代表的代谢类药物，联合IL-17抑制剂的临床试验结果显示：针对合并肥胖的PsA患者，联合方案可同时实现关节症状改善与体重下降，双重获益显著。中轴型PsA（AxPsA）的特征 AXIS研究发现，AxPsA与经典的强直性脊柱炎（AxSpA）在表型上存在差异：患者更年轻、男性更多、HLA-B27阳性率更高、炎性背痛更常见。影像学特点：AxPsA的骶髂关节（SIJ）骨髓水肿（BMO）更多位于上骶骨，且对生物制剂治疗反应良好。然而，无BMO但临床高度怀疑的中轴型PsA也被正式识别，强调诊断不能仅依赖MRI。一项研究显示，阿普斯特（Apremilast）治疗中轴症状的MRI评分（SPARCC）无明显改善，提示其对抗中轴炎症效果可能有限。难治性PsA与疾病预防随着治疗手段增多，难治性PsA的标准化定义、分层及银屑病向PsA转化的早期防控成为研究热点，GRAPPA、EULAR两大国际组织相继发布共识标准。 1.复杂性难治PsA（C2M-PsA）：存在持续PsA相关症状，对≥2种生物/靶向合成DMARD（b/tsDMARDs）应答不佳，患者或医师判定疾病控制不理想，无需强制客观炎症证据。 2.治疗抵抗性PsA（TR-PsA）：核心标准更为严格，需同时满足：持续症状、对≥3种不同作用机制的PsA药物（含≥2种b/tsDMARDs）应答不足、患者与医师共同判定疾病活动、必须存在客观炎症证据，且排除纤维肌痛、骨关节病等其他疾病干扰。两类分层标准为临床评估疾病严重程度、制定挽救治疗方案提供统一依据。 2016年起，PsA研究正式迈入疾病预防新时代，核心目标是阻断银屑病向关节炎进展，目前相关探索已取得阶段性成果。一、银屑病进展为PsA的风险预测队列研究证实，高BMI、高龄、多线生物制剂序贯使用，是银屑病患者继发PsA的独立高危因素。而临床常用的活动性腱鞘炎超声征象，并非PsA有效预测指标，提醒临床勿过度解读亚临床影像异常。不同靶点药物的预防效果存在差异：银屑病患者一线生物制剂治疗失败后，换用IL-17抑制剂与IL-23抑制剂，在预防关节炎发生方面效果相当；TNF抑制剂可轻度降低银屑病患者进展为PsA的累积风险，但证据仍需进一步验证。二、AI预测技术成为未来方向依托人工智能技术，可整合临床资料、遗传信息、影像数据、生物标志物、皮损表现、用药史等多维度数据，构建PsA风险预测模型，精准筛选高危银屑病患者，实现早期干预、提前预防。目前该方向处于探索阶段，也是未来重点研发领域。总结从1956年Wright的经典描述，到2026年单细胞水平的精细解析，银屑病关节炎研究正经历从”黑箱”到”白箱”的范式转变。McGonagle教授的演讲提醒我们：理解疾病的微观免疫地理学，是实现宏观临床精准管理的前提。 McGonagle教授最后总结： 1. Soligenix Announces Recent Updates and First Quarter 2026 Financial Results

PRINCETON, N.J., May 8, 2026 /PRNewswire/ -- Soligenix, Inc. (Nasdaq: SNGX) (Soligenix or the Company), a late-stage biopharmaceutical company focused on developing and commercializing products to treat rare diseases where there is an unmet medical need, announced today its recent updates and financial results for the quarter ended March 31, 2026. "We remain disappointed with the unanticipated outcome of the FLASH2 (Fluorescent Light Activated Synthetic Hypericin 2) study," stated Christopher J. Schaber, PhD, President and Chief Executive Officer of Soligenix. "Despite the fact that HyBryte™ (synthetic hypericin) demonstrated statistically significant reductions in cutaneous T-cell lymphoma (CTCL) lesions after 6 weeks treatment in the first FLASH study, a similar signal was not observed with 18 weeks of treatment in this study. Over the coming weeks, we will analyze the data to better determine why the study did not meet expectations. If there is any clarity gained from further analysis of the dataset, especially with respect to specific subsets of patients that may benefit from HyBryte™ therapy, then we intend to communicate our findings and explore follow-up discussions with the European Medicines Agency (EMA) and the Food and Drug Administration (FDA)." To view this press release, please click here. Financial Results – Quarter Ended March 30, 2026 Soligenix had no revenue or related costs for the quarter ended March 31, 2026 and 2025, respectively. Soligenix's net loss was $2.8 million, or ($0.28) per share, for the quarter ended March 31, 2026, compared to $3.0 million, or ($0.97) per share, for the same prior year period. This decrease in net loss was primarily due to a decrease in operating expenses. Research and development expenses were $1.8 million as compared to $1.9 million for the quarter ended March 31, 2026 and 2025, respectively.
